How to Check Your Benefits
We do not verify coverage on your behalf, but we make it easy for you to confirm your benefits before your first session.
Start by downloading our insurance form, which includes a clear list of questions to ask your insurance provider. Then, call the member services number listed on the back of your insurance card and use the form to guide the conversation. This helps you find out:
Whether nutrition counseling is covered
How many sessions are included per year
What your copay, deductible, or coinsurance might be
Whether prior authorization or a referral is required
Many of our clients find this step empowering and helpful for avoiding surprise bills.

I’ve been working with Jaquelyn and she’s fantastic! Her knowledge, teachings and emotional support have improved the quality of my life. I’ve been able to face my eating difficulties and am learning how to better manage them. I highly recommend her!
